I acknowledge that I am registering myself for this program voluntarily. I understand and agree to the Covid-19 safety protocols and will release, indemnify, and hold harmless the Middletown Arts Center, the Middletown Twp. Cultural and Arts Council, the Township of Middletown, and the Middletown Reformed Church as well as their officers, volunteers, and employees, of any liability resulting from the unintentional exposure to Covid-19 while participating.
I will not attend the program if anyone in my household is sick, under quarantine, or if we have traveled to any high-risk areas within the past four weeks.
I understand that if I do not follow safety protocols as directed, I may be prevented from participating in the program and no refund will be given.
