By purchasing tickets to this event, I understand and agree to the COVID-19 safety protocols and will release, indemnify, and hold harmless the Middletown Arts Center, the Middletown Twp. Cultural and Arts Council, the Township of Middletown, and the Middletown Reformed Church as well as their officers, volunteers, and employees, of any liability resulting from the unintentional exposure to COVID-19 while participating.
By attending this event, I acknowledge that I am not sick and have not been in contact with anyone with symptoms or a confirmed case of COVID-19 within the past 10 days. I will not attend the program, nor will anyone in my household/party, if anyone from my household/party is sick, under quarantine, or if we have traveled to any high-risk areas within the past four weeks.
If I or anyone in my household/party tests positive for, or exhibits symptoms of, COVID-19 at any time within 10 days of my attended performance, I will contact the Box Office at 732-706-4100.
I understand that if I do not follow safety protocols as directed, I may be prevented from attending the program and no refund will be given.
